What is the Abilities First DD Health Home?
Discover your health needs and goals with the guidance of our dedicated healthcare professionals who promote wellness. With the assistance of a Support Coordinator, you can tailor your journey to better health without impacting your Medicaid benefits. Participation is entirely your choice, and best of all, the program is funded by Medicaid, ensuring it is free for you.
What a DD Health Home is NOT:
A health home is not a place to live, nor does it provide direct medical services or replace your doctor. Instead, it serves as an additional partner to complement your existing planning team, enhancing your overall healthcare experience.
What’s Next?
The DD Health Home program began in July 2024, offering a seamless way to enhance your healthcare journey. Eligible individuals will be automatically enrolled with a local health home provider, with no authorizations required to join. For more details, simply talk to your Support Coordinator. Participation is flexible—you can leave the program at any time or switch DD Health Home providers as needed.
