What is Steps Forward?

Steps Forward/DD Health Home Services is a team of caring health professionals who support you in staying healthy and reaching your health goals. Whether you want to manage a condition or prevent one from developing, we’re here to help. A health home is not a place to live, instead, our team members serve as additional partners to complement the support of your existing care team. You’ll be assigned a Nurse Care Manager who will understand your health needs, provide education on your health conditions, and who will work with you and your current care team to create a plan for better health. This support does not impact or change your Medicaid benefits, nor does it replace any services you currently receive!

Whether you want to

• eat better,
• move more,
• manage a condition like diabetes,
• stop smoking,
• or just feel better and stay healthy,

Steps Forward/ DD Health Home team is ready to help you make a plan and stick with it. You will have access to interactive health videos, support groups, and classes to help you stay on track with your wellness. Your Nurse Care manager will connect you to healthcare providers and services when you need that support. And guess what? Your Steps Forward/DD Health Home team is still here for you even after you reach your goals!
